The Connecticut Community Oral Health Systems Development Project (CCOHP) is funded by the HRSA/Community Integrated Service System Program (CISS) from September 30, 1997 through October 31, 2001. The project has an unfunded extension through October 31, 2002. CCOHP was one of only two oral health program grantees, nationwide, from among approximately 150 CISS-Community Organization Grants Program's during that time. The mission of CCOHP is to enhance oral health and oral health care access through the development of community health care delivery systems, and through the integration of oral health into local health and human services programs. The goals of CCOHP are to: 1) Help develop community-based collaborations (coalitions) comprised of broad representation of stakeholders from the public and private sectors; 2) Facilitate the activities of and provide technical assistance to the collaborations in their efforts to define project mission, goals & objectives; conduct needs & resources assessments; strengthen collaborations with local health and human services professionals; develop strategic, business & implementation plans; apply for funding and other support; address statutory, regulatory, & contractual issues; 3) Conduct annual networking and educational conferences; 4) Standardize and centralize data collection. The project had a budget of $50,000 each year for 4 years: 50% for expert consultant(s), 40 percent for grants-in-aid to CCOHP community sites, and 10 percent for annual conference and other costs. Thirteen CCOHP sites have been developed throughout the State. Many of the collaborations have fully achieved the project goals. A broad range of safety-net facilities and health systems have evolved from CCOHP throughout the State, including: new school-based dental clinics, new community health center dental clinics, new store-front dental clinics, three mobile dental clinics, multi-town and multi-service system collaborations, and more.
