Best Practice Approach Example -

In 2001, the Arizona Department of Health Services, Office of Oral Health developed and implemented a mini-grant and technical assistance program to assist communities in preventing oral diseases and increasing access to oral health services. Community organizations, including county health departments, school departments, nonprofit organizations and Native American tribes, are eligible to apply for the Healthy Arizona 2010 Community Support Grants. Small grants in the amount of $500-5000 are awarded to assist communities to either (1) conduct a locally-designed oral health needs assessment, (2) develop an oral health improvement plan, or (3) implement an oral health improvement project. Each organization must have completed an oral health needs assessment in order to apply for funding to develop a strategic plan. To apply for funding to implement a local project, both a needs assessment and a strategic plan must be completed. Grantees are encouraged to follow a community development guide provided by the Office of Oral Health and are provided with technical assistance from the Office of Oral Health staff. Organizations conducting on oral health needs assessment or developing a strategic plan must have an identifiable community group to direct the oral health improvement efforts. Of the communities funded for up to $5,000 each, significant progress has been made in such as acquiring a grant to implement a county-based dental pubic health program, completing a local assessment and prioritization of oral health needs, designing and acquiring funding for a case management dental program to be implemented through school-based health centers.
